My name is Mike **** my # 925-493-0982 my father paid into Cal Pers long term care for 40 years this was his go to plan for caring for himself in the case of not being able to.
He Charles Robert Swift coverage id#41-118586 was diagnosed with stage 4 cancer in jan 2 2016 the cal pers rep came to are house jan 19th and approved him for care as he could not ambulate aside from eating he was helpless! besides going to chemo appointments and doctors etc. my father with my help made endless calls and received no less than 20 letters saying there was missing papers certificates doctors signed papers etc all of which was given to the cal pers rep on the first visit and also faxed as well as verbally given by the social worker assigned to my fathers case. the final straw are care giver was said to need a special care giver certificate for payment approval they said this was the only thing left she took the 2hr online coarse I paid the $90 submitted the certificate on April 13th my father passed on April 16th they then said he only qualified for 3 days of coverage since that time I have submitted 7 time cards for the 3 day period the certificate 5 times the direct deposit slip for his account 5 times talked to no less then 10 people who wanted to talk with my father (so do I ) all this after giving them the death certificate in April .
STILL NO PAYMENT. OMG.. DO NOT PAY OR KEEP CAL PERS LONG TERM CARE
